Established Patient Care
Established In-Home Visit ($150.00)

Please note that all services provided require a $50.00 deposit in order to secure your appointment. All deposits go toward your final invoice.
All In-person appointments include an additional $50.00.
All patients are subject to an additional fee of 3% if they decide to complete all payments via credit card.

Prior to our appointment, please complete the paperwork. I will send it to your email and or cell phone. If you need me to, I can send it via mail. Please sign off on the consent form for your medical history. This will help me get an accurate picture of you.
